New eSignature form now available for more flexibility

As we continue in our efforts to make doing business with your clients as efficient as possible, the Financial Changes Request for Life and Health Policies form has now been made available as a digital eSignature form.

With this form, you can easily assist your client in submitting a request for a cash loan, insurance policy surrender value or dividend withdrawal.   

Change requests can be submitted quickly and easily through the My Advisor Dashboard. The eSignature forms section on the dashboard gives you access to online forms and applications that can be signed digitally, giving you a more convenient way to work with your clients.

You will receive a copy of the form once all parties have signed. Please note that your MGA may require a copy of the completed and signed document for auditing purposes. If this applies to you, please ensure to forward the signed document to your MGA.

The following eSignature forms are already available to you on My Advisor Dashboard:

• C-0050         Transfer of Ownership Form
• C-0170(a)     Insurance Electronic PAD Agreement
• D-0017         Beneficiary Designation Form
• INP-125A     Investment Change Form Empire Life Class, GIF contracts
• INP-125C     Investment Change Form for Elite or Elite XL 

Updates will be provided as more forms and applications are enabled for eSignature.
